Special Needs Accomodation Process (SNAP)

Wait List Procedures

Program Enrollment Packets

Child Development Center (Full Day, Hourly, Part Day Preschool, Family Child Care) & School Age Care print or complete:

• Army CYSS Parent Central Services Office Registration Checklist
• Army CYSS Health Screening Tool
• DD 2652 Application for DOD Child Care Fees
• Health Assessment (30 days to have completed)
• SNAP paperwork if applicable
• Official Shot Record with updated immunizations

Middle School/Teen Program:
• Army Child and Youth Services Health Screening Tool
• SNAP paperwork if applicable

Youth Sports:
• Army Child and Youth Services Health Screening Tool
• Health Assessment/Sports Physical – All participants must have a current CYSS membership,
shot records and a sports physical that will be current for the duration of the program at time of registration
• SNAP paperwork if applicable
• Official Shot Record with updated immunizations

SKIES Unlimited (Instructional Programs):
• Army Child and Youth Services Health Screening Tool
• SNAP paperwork if applicable

FMWR  facilities are for eligible personnel only.
Active duty and retired military personnel, Air National Guard and Reserve personnel, family members of the above, DoD and retired civilian employees, and cadets are eligible to use MWR facilities. Proper identification may be requested by staff.
